Using MEMO-Snap
- Jot down patient and caller info as the call or message comes in. If the telephone encounter meets criteria for a billable service there will be no need to back track to capture the needed information later on. MEMO-Snap can also be used by non-physician practitioners to document and report telephone care.
- Use the back of the form to record the details of your telephone encounter. This information can then be transcribed or scanned into the patients medical record for documentation purposes. Later on, with this information already charted, you will not have to repeat it if a face-to-face visit results from the phone call. Just link your progress note to the telephone encounter…and carry on.
- Enter your assessment or diagnosis in the field on the front of the form. Record the amount of time spent in medical discussion with your patient in the space provided, If the encounter meets criteria for a billable service, choose the appropriate CPT code from the list. Use check boxes to indicate the type of follow-up required by support staff.
- Store completed forms in the pocket. When you return to office staff for follow-up, charting, and billing.

- TIP: Use MEMO-Snap to gather utilization data on the frequency and complexity of telephone care provided by you and your practice. At the end of several months, you may be surprised at the amount of revenue that has been left ‘on hold’!
- TIP: Even brief calls, or calls that are not billable or separately reportable for reimbursement can include information important to a patient’s care, and which should be included in the chart. If the patient’s chart is not at hand, use MEMO-Snap to indicate that you spoke to the patient to report or discuss test results, or made a change in medication dosage, and then pass the slip along to support staff to chart the information for you.
